ตามที่ข้าพเจ้านางสาวอาริญภร ต๊ะวิจิตร ที่อยู่บ้านเลขที่ ๙๘/๑ ตำบลขัวมุง อำเภอ สารภี จังหวัด เชียงใหม่ได้ให้เช่าอาคารและอุปกรณ์ คือ อาคารเลขที่ ๓๘๘/๑ ถนนเจริญราษฎร์ ตำบลฟ้าฮ่าม อำเภอเมือง จังหวัดเชียงใหม่ กับท่าน(ผู้เช่า)รายละเอียดปรากฏตามสัญญาเช่าอาคารและอุปกรณ์ ฉบับลงวันที่.................. เดือน...........................พ.ศ. .................. ซึ่งต่อไปจะเรียกว่า “สัญญา” นั้นเนื่องจากท่านปฏิบัติผิดนัดผิดสัญญาเช่าดังกล่าว โดยท่านผิดสัญญาเช่าข้อ 2 และทำให้สัญญาเช่าเลิกกันทันทีตามข้อ ๔ ของสัญญาเช่านั้น ซึ่งข้าพเจ้าได้บอกกล่าวให้ท่านแก้ไขเยียวยาการผิดสัญญาของท่านในเวลาอันเหมาะสมแล้ว แต่ท่านยังคงเพิกเฉย ทำให้ข้าพเจ้า(ผู้ให้เช่า)ได้รับความเสียหายและทำให้สัญญาสิ้นสุดลงทันที
โดยหนังสือนี้ ข้าพเจ้าขอบอกเลิกสัญญากับท่านโดยให้สัญญามีผลสิ้นสุดลงนับแต่วันที่ท่านได้รับหนังสือฉบับนี้ และให้ท่านชดใช้ค่าเสียหาย ( หากมี ) รวมทั้งส่งมอบทรัพย์สินของข้าพเจ้าที่ท่านครอบครองคืนให้แก่ข้าพเจ้า หรือบุคคลที่ข้าพเจ้ากำหนดในสภาพเรียบร้อย และปฏิบัติตามสัญญาเช่าโดยเคร่งครัด ภายในวันที่
อนึ่ง ข้าพเจ้าขอสงวนสิทธิเรียกค่าเสียหายที่ข้าพเจ้า ตรวจพบจากการกระทำของท่านในอนาคตอีกโสดหนึ่งด้วย
