โครงการจัดการโรคเบาหวาน – โรคความดันโลหิตสูง จัดทำขึ้นเพื่อแก้ไขปัญหาโรคเรื้อรังในชุมชน มีวัตถุประสงค์ดังนี้ 1. เพื่อตรวจคัดกรองกลุ่มเสี่ยงโรคเบาหวาน โรคความดันโลหิตสูง 2. เพื่อให้ผู้ป่วยโรคเบาหวานโรคความดันโลหิตสูง มีความรู้และมีทักษะในการดูแลตนเองที่ถูกต้องและเหมาะสม 3. เพื่อให้ครอบครัวมีส่วนร่วมในการดูแลผู้ป่วยเบาหวาน รวมถึงการปรับเปลี่ยนพฤติกรรมที่เป็นความเสี่ยงต่อโรคของคนในครอบครัวต่อการเกิดโรคใหม่ กลุ่มเป้าหมาย คือ ประชาชนอายุ 35 ปี ขึ้นไป และกลุ่มผู้ป่วยโรคเรื้อรังที่ไม่สามารถดูแลตนเองได้ตามระดับปกติ ระยะเวลาดำเนินโครงการ เดือนกันยายน 2558 ได้รับงบประมาณจาก จากกองทุนสุขภาพระดับพื้นที่เทศบาลภูผาแดง จำนวน 56,050 บาท ผลที่คาดว่าจะได้รับ ลดอัตราการเกิดโรคเรื้อรังผู้ป่วยรายใหม่ในชุมชน มีระบบ และรูปแบบการดูแลผู้ป่วยโรคเรื้อรังแบบองค์รวมที่มีคุณภาพ และสามารถนำไปปรับใช้ในการดูแลผู้ป่วยกลุ่มอื่นๆต่อไปได้อย่างเหมาะสม
