เรื่อง รับสมัครบุคคลพลเรือนเพื่อสอบคัดเลือกเป็นลูกจ้างชั่วคราวรายเดือน



ด้วย กรมแพทย์ทหารอากาศ มีความประสงค์จะรับสมัครบุคคลพลเรือนเพื่อสอบคัดเลือกเป็นลูกจ้างชั่วคราวรายเดือน จำนวน ๔ คน มีรายละเอียดดังนี้

๑. ตำแหน่งและคุณวุฒิ
๑.๑ พนักงานธุรการ ระดับสนับสนุน ๓ วิทยาลัยพยาบาลทหารอากาศ
รับปริญญาตรี วิทยาการจัดการ (สาขาบริหารธุรกิจ,บริหารทรัพยากรมนุษย์)
๑.๒ พนักงานธุรการ ระดับสนับสนุน ๑ แผนกส่งเสริมสุขภาพ กองเวชศาสตร์ป้องกัน
รับประกาศนียบัตรวิชาชีพ (สาขาพณิชยการ)
๑.๓ ผู้ช่วยช่างทั่วไป ระดับช่าง ๑ โรงพยาบาลภูมิพลอดุลยเดช (ปฏิบัติงานที่ กองเวชบริภัณฑ์ กรมแพทย์ทหารอากาศ)
รับมัธยมศึกษาตอนต้น - มัธยมศึกษาตอนปลาย (สายสามัญ)

๒. คุณสมบัติของผู้สมัคร
๒.๑ มีอายุไม่ต่ำกว่า ๑๘ ปีบริบูรณ์ (นับถึงวันบรรจุ)
๒.๒ กรณีเป็นชายต้องพ้นภาระการเกณฑ์ทหารแล้ว
๒.๓ มีสัญชาติไทย แต่มิใช่โดยการแปลงสัญชาติตามกฎหมาย
๒.๔ เป็นผู้เลื่อมใสในการปกครองระบอบประชาธิปไตย อันมีพระมหากษัตริย์ทรงเป็นประมุขตามรัฐธรรมนูญแห่งราชอาณาจักรไทยด้วยความบริสุทธิ์ใจ
๒.๕ ไม่เป็นผู้ดำรงตำแหน่งกำนัน แพทย์ประจำตำบล สารวัตรกำนัน ผู้ใหญ่บ้านและผู้ช่วยผู้ใหญ่บ้าน
๒.๖ ไม่เป็นผู้ดำรงตำแหน่งข้าราชการการเมือง
๒.๗ ไม่เป็นผู้มีร่างกายทุพพลภาพจนไม่สามารถปฏิบัติหน้าที่ได้ไร้ความสามารถหรือมีจิตฟั่นเฟือนไม่สมประกอบ
๒.๘ ไม่เป็นผู้อยู่ในระหว่างถูกสั่งให้พักราชการหรือถูกสั่งในออกจากราชการไว้ก่อน
๒.๙ ไม่เป็นผู้บกพร่องในศีลธรรมอันดีจนเป็นที่รังเกียจของสังคม
๒.๑๐ ไม่เป็นกรรมการพรรคการเมือง หรือเจ้าหน้าที่พรรคการเมือง
๒.๑๑ ไม่เป็นบุคคลล้มละลาย
๒.๑๒ ไม่เป็นผู้เคยต้องโทษจำคุกโดยคำพิพากษาถึงที่สุดให้จำคุกเพราะกระทำความผิดทางอาญาเว้นแต่เป็นโทษสำหรับความผิดที่ได้กระทำโดยประมาท หรือความผิดลหุโทษ
๒.๑๓ ไม่เป็นผู้เคยถูกลงโทษให้ออก ปลดออก หรือไล่ออกจากรัฐวิสาหกิจหรือหน่วยงานอื่นของรัฐ
๒.๑๔ ไม่เป็นผู้เคยถูกลงโทษให้ออกหรือปลดออกเพราะกระทำผิดวินัย
๒.๑๕ ไม่เป็นผู้เคยถูกลงโทษไล่ออกเพราะกระทำผิดวินัย
๒.๑๖ ไม่เป็นผู้เคยกระทำการทุจริตในการสอบเข้ารับราชการ

๓. หลักฐานการสมัคร
๓.๑ บัตรประจำตัวประชาชน พร้อมสำเนา ๑ ฉบับ
๓.๒ รูปถ่ายหน้าตรงไม่สวมหมวกขนาด ๑ นิ้ว จำนวน ๓ รูป
๓.๓ ต้นฉบับคุณวุฒิการศึกษา และระเบียนแสดงผลการเรียนพร้อมสำเนาอย่างละ ๑ ฉบับ
๓.๔ สำเนาทะเบียนบ้านของผู้สมัคร บิดามารดาของผู้สมัคร (หากบิดาหรือมารดาถึงแก่กรรมต้องนำสำเนามรณบัตรของบิดาหรือมารดาหรือหนังสือรับรองของนายทะเบียนท้องถิ่นแสดงสัญชาติของบิดาหรือมารดาไปแสดงด้วย) พร้อมสำเนาอย่างละ ๑ ฉบับ
๓.๕ กรณีเป็นชายให้นำสมุดประจำตัวทหารกองหนุน (สด.๘) หรือทะเบียนกองประจำการ (สด.๓) หรือใบรับรองผลการตรวจเลือกทหารกองเกินเข้ารับราชการกองประจำการ (สด.๔๓) พร้อมสำเนา ๑ ฉบับ
๓.๖ ผู้สมัครที่เป็นหญิงหากทำการสมรสแล้วให้ถ่ายสำเนาใบสำคัญการสมรสไปแสดงด้วย จำนวน ๑ ฉบับ

๔. สถานที่รับสมัคร
สมัครด้วยตนเองได้ที่ กองกำลังพล กรมแพทย์ทหารอากาศ ถนนพหลโยธิน แขวงคลองถนน เขตสายไหม กรุงเทพมหานคร โทร. ๐ ๒๕๓๔ ๒๕๗๐ ในเวลาราชการทุกวัน ตั้งแต่วันที่ ๒๙ กันยายน – ๘ ตุลาคม ๒๕๕๗

๕. การแต่งกาย
ผู้ที่สมัครเข้ารับการสอบคัดเลือก จะต้องแต่งกายให้สุภาพเรียบร้อย (งดสวมเสื้อยืด กางเกงขาสั้น และสวมรองเท้าแตะโดยเด็ดขาด) ตั้งแต่เข้ารับการสมัคร การสอบภาควิชาการ และการสอบสัมภาษณ์ กรณีที่แต่งกายไม่เหมาะสมให้ถือว่าเป็นผู้ที่ขาดความรับผิดชอบและขาดความพร้อมเบื้องต้นในการปฏิบัติหน้าที่ตำแหน่งที่เข้ารับการสอบคัดเลือก ซึ่งจะพิจารณาตัดสิทธิ์ในการสมัครเข้ารับการสอบคัดเลือกทุกขั้นตอน
