PI: healthy, no previous admission
FH: Husband died of accident, no kid
Vital signs: T 37.8 0C, P 100 BPM, RR 24 CPM, BP 140/90 mmHg, pain score 7
Physical examination
HEENT : WNL
CVS : mild dyspnea, both lung- clear, Normal S1, S2 no mur mur
Abdomen : round, mild tenderness at RLQ, bowel sound 8 /min, Liver, spleen – not palpable, typany sound all quadrants, No CVA tenderness, femoral LN – not enlarged
Motor power grade V all limbs, full ROM, CN- intact
Initial diagnosis : Acute appendicitis
Doctor’s order
- NPO stat. on 5%DNSS 1000 ml IV drip @100 cc/hr
- CBC, e’lyte, UA stat.
- Set OR for appendectomy at 4 PM
- Retained foley’s catherter for OR
