อุทยานแห่งชาติภูสอยดาว แต่เดิมเป็นวนอุทยานภูสอยดาว ได้สำรวจจัดตั้งเป็นวนอุทยานภูสอยดาว โดยสำนักงานป่าไม้เขตพิษณุโลก เมื่อวันที่ 13 พฤศจิกายน พ.ศ. 2533 มีพื้นที่เพียง 20,000 ไร่ จนกระทั่งปีงบประมาณ พ.ศ. 2535 กรมป่าไม้ได้จัดสรรงบประมาณให้สำนักงานป่าไม้เขตพิษณุโลกทำการสำรวจพื้นที่ เพิ่มเติมเพื่อผนวกเข้ากับพื้นที่เดิมของวนอุทยานภูสอยดาว ผลการสำรวจพื้นที่เพิ่มเติมในเขตป่าสงวนแห่งชาติป่าน้ำปาด ท้องที่อำเภอน้ำปาด จังหวัดอุตรดิตถ์ และในเขตป่าไม้ถาวรตามป่าภูสอยดาวท้องที่อำเภอน้ำปาด จังหวัดอุตรดิตถ์ป่าภูสอยดาว ท้องที่อำเภอชาติตระการ จังหวัดพิษณุโลก ตามมติคณะรัฐมนตรีได้เนื้อที่รวม 48,962.5 ไร่ หรือ 78.34 ตารางกิโลเมตร

ต่อมาในเดือนสิงหาคม พ.ศ. 2536 สำนักงานป่าไม้เขตพิษณุโลกได้ขอจัดตั้งวนอุทยานภูสอยดาวเป็นอุทยานแห่งชาติภูสอยดาว ได้รายงานให้กรมป่าไม้ทราบว่า พื้นที่วนอุทยานภูสอยดาวเป็นพื้นที่ที่มีสภาพป่าค่อนข้างสมบูรณ์ จึงเห็นสมควรที่จะรักษาพื้นที่ป่าแห่งนี้ไว้เป็นพื้นที่ป่าอนุรักษ์ และพื้นที่ใกล้เคียงยังมีสภาพป่าธรรมชาติที่สมบูรณ์ สามารถผนวกเป็นเขตอุทยานแห่งชาติได้อีกเป็นจำนวนมาก โดยกำหนดให้เป็นอุทยานแห่งชาติภูสอยดาว จนกระทั่งปี 2551 ได้มีพระราชกฤษฎีกากำหนดให้เป็นอุทยานแห่งชาติ ซึ่งประกาศในราชกิจจานุเบกษา เล่ม 125 ตอนที่ 71 ก ลงวันที่ 28 พฤษภาคม พ.ศ. 2551[3]
