โรคเยื่อตาอักเสบ

เยื่อตา หรือเยื่อบุตา (Conjunctiva) เป็นเยื่อเมือก (Mucous membrane) บางๆ บุเปลือกตา/หนังด้านในที่เรียก Palpebral conjunctiva และส่วนที่บุตาขาว (Sclera) เรียกว่า Bulban conjuctiva โดยรอยต่อระหว่างเยื่อตาส่วน Palpebral และส่วน Bulbar conjuctiva มาจรดกัน จะเป็นส่วนของร่องตา เรียกว่า Fornix
ทั้งนี้ ใต้เยื่อตาของคนเรา มีเซลล์/เนื้อเยื่อที่ให้ภูมิคุ้มกันต้านทานโรค คือ เนื้อเยื่อในระ บบน้ำเหลือง และเซลล์เม็ดเลือดขาว ชนิด Lymphocyte แต่ถึงแม้จะมีเซลล์/เนื้อเยื่อที่ให้ภูมิ คุ้มกันต้านทานโรคก็ตาม เยื่อบุตายังมีโอกาสเกิดการอักเสบได้จากหลายสาเหตุ อาจเป็นกับตาข้างเดียวหรือ 2 ตาพร้อมกัน หรือจากตาข้างหนึ่งลามไปอีกข้างในเวลาต่อมาก็ได้


อาการของเยื่อตาอักเสบ ได้แก่
• ตาแดง จะเห็นชัดบริเวณที่เยื่อตาคลุมตาขาวอยู่ ทำให้เห็นชัดว่าตาขาวแดง อาจจะแดงมากในรายติดเชื้อแบคทีเรียแดงเรื่อๆในรายอักเสบจากภูมิแพ้
• มีขี้ตา ด้วยเหตุที่เยื่อตาปกติสร้างน้ำตาชนิดเป็นเมือก (Mucous) หากมีเซลล์อักเสบปะปนออกมา จะทำให้น้ำตาออกมาเป็นขี้ตา ลักษณะของขี้ตาอาจบ่งถึงต้นเหตุของการอักเสบ เช่น เป็นน้ำใสๆ หรือน้ำตาอย่างเดียว มักจะติดเชื้อไวรัสถ้าเป็นเส้นเมือกๆ อาจเป็นจากภูมิแพ้ ถ้าแฉะคล้ายหนองข้น มักจะเป็นการติดเชื้อแบคทีเรีย เป็นต้น
• เจ็บตา เคืองตา คล้ายมีผงอยู่ในตา มักจะไม่ค่อยมีอาการปวดมากนัก
• น้ำตาไหล
• บางรายอาจมีอาการในระบบทางเดินหายใจร่วมด้วย เช่น เจ็บคอ เป็นโรคหวัด บ่งว่าน่าจะเกิดจากเชื้อไวรัส
• ที่สำคัญ คือ การมองเห็นยังปกติดี หรืออาจพร่ามัวบ้าง เพราะมีน้ำตาและขี้ตามาบัง เช็ดออกแล้วสายตาจะปกติ
ทั้งนี้สิ่งที่เราจะมองเห็นได้ หรืออาการแสดงจากมีเยื่อตาอักเสบ คือ
• ลักษณะตาแดง จะแดงมากทางด้านร่องตา และค่อยๆจาง เมื่อมาถึงรอยต่อระ หว่างตาขาวกับตาดำ (Limbus) เรียกกันว่า Conjunctival injection ต่างจากตาแดงจากเนื้อ เยื่อในลูกตาที่เรียกว่า Ciliary injection ซึ่งจะแดงมากบริเวณ Limbus แล้วค่อยๆจางไปทางร่องตา ซึ่งเป็นอาการตาแดงที่พบในโรคตาที่รุนแรงกว่า เยื่อบุตาอักเสบ เช่น กระจกตาอักเสบ ต้อหินเฉียบพลัน ม่านตาอักเสบ ทั้งนี้เพราะการแดงจากเยื่อบุตาอักเสบ เป็นการขยายของหลอดเลือดที่มาเลี้ยงเยื่อบุตา ซึ่งอยู่ผิวๆ และมาจากด้านร่องตาสู่ limbus เชื้อที่ก่อโรคเยื่อบุตาบางชนิดอาจก่อให้เกิด จุดเลือดออก (Petichael haemorrhage) ในตาขาว (เห็นเป็นจุดแดงๆ กระจายในตาขาว) หรือบางรายมีเลือดออกเป็นปื้นในตาขาว(เลือดออกใต้เยื่อตา/Subconjunc tival haemorrhage) ก็ได้ ยิ่งทำให้รู้สึกตาแดงเข้มมากขึ้น
• เยื่อตาบวม หนาขึ้นกว่าปกติ บางรายอาจพบมีน้ำซึมออกจากหลอดเลือดที่โป่งพอง ส่งผลให้เกิดตาบวม (Chemosis)
• อาจพบเป็นตุ่ม/ปุ่มที่เรียกว่า ตุ่ม Papilla มักพบที่เยื่อบุตาใต้หนังตา เป็นการตอบ สนองของเยื่อบุตาต่อการอักเสบที่ไม่เจาะจงว่ามีสาเหตุจากอะไร เป็นตุ่มขนาดเล็ก เล็กกว่า 1 มม. พบได้ในเยื่อบุตาอักเสบหลายชนิด บางครั้งตุ่มนี้อาจรวมกันจนมีขนาดใหญ่ขึ้นมากได้ ดังที่พบในภาวะที่เรียกว่า GPC (Giant papillary conjunctivitis) ตุ่ม Papilla จะมีจุดแดงตรงกลางตุ่ม
• อาจพบตุ่ม/ปุ่มอีกชนิดที่เรียก ตุ่ม Follicle ขนาดใหญ่กว่า ตุ่ม Papilla พบเจาะจงในโรคบางชนิด เช่น การติดเชื้อไวรัสการแพ้ยา ริดสีดวงตา เป็นต้น ตัวตุ่ม Follicle จะไม่มีตุ่มแดงตรงกลาง แต่จะแดงบริเวณขอบรอบตุ่ม (ใช้แยกจากตุ่มPapilla)
• อาจจะพบเป็นแผลอักเสบที่เรียกว่า Membrane มีทั้งชนิด True membrane ซึ่งถ้าพยายามลอกออกจะมีเลือดออก บ่งถึงมีการอักเสบรุนแรง หรือชนิด Pseudo membrane ที่เป็นแผ่นคล้ายกัน แต่ถ้าพยายามลอกออก จะไม่มีเลือดออก บ่งถึงการอักเสบที่ไม่รุนแรง
