1. ประชากรคือผู้สูงอายุโรคเรื้อรังในความรับชอบของศูนย์แพทย์ชุมชนเมือง1 (หัวทะเล) ตำบลในเมือง อำเภอเมืองจังหวัดนครราชสีมาจำนวน 114 คนโดยมีคุณสมบัติดังนี้
1.1 เกณฑ์การคัดเข้า (Include criteria)
1.1.1 เป็นผู้สูงอายุทั้งชายและหญิงอายุ 60 ปีขึ้นไป
1.1.2 ป่วยด้วยโรคเบาหวานและหรือโรคความดันโลหิตสูงมาไม่น้อยกว่า 3 เดือน
1.1.3 มีชื่ออยู่ในความรับชอบของศูนย์แพทย์ชุมชนเมือง1 (หัวทะเล)
1.1.4 มีสติสัมปชัญญะสมบูรณ์สามารถเข้าใจและสื่อสารภาษาไทยได้
1.1.5 ยินดีตอบแบบสัมภาษณ์และแบบคัดกรอง
2. กลุ่มตัวอย่างในการศึกษาวิจัยในครั้งนี้ผู้วิจัยได้กำหนดขนาดกลุ่มตัวอย่าง โดยใช้เกณฑ์การประเมินค่าจากจำนวนประชากร
