การขออนุมัติออกไปปฏิบัติงานนอกสถานที่ ต้องขออนุมัติจากผู้บังคับบัญชาขั้นสูงกว่าขึ้นไป และฝ่ายบุคคล ตามลำดับ ก่อนออกไปปฏิบัติงานนอกสถานที่ทุกครั้ง
กรณีผู้บังคับบัญชาขั้นสูงกว่าขึ้นไปไม่อยู่ ให้ระบุสาเหตุในช่องเซ็นชื่อและส่งแบบฟอร์มให้ฝ่ายบุคคลอนุมัติต่อไป
