ทุกข์ หรือ ทุกขัง (บาลี: ทุกฺข) เป็นหลักธรรมสำคัญในพระพุทธศาสนา แปลว่าทนอยู่ในสภาพเดิมได้ยาก โดยทั่วไปหมายถึง สังขารทั้งปวง อันได้แก่ ขันธ์ 5 ซึ่งเป็นที่ตั้งของกองทุกข์

ในทางพระพุทธศาสนา การกำหนดทุกข์ถือเป็นกิจในอริยสัจ 4 ที่ชาวพุทธต้องทำเพื่อละและปล่อยวางในลำดับต่อไปในกิจทั้งสี่ในอริยสัจ อันได้แก่ รู้ทุกข์ เพื่อค้นหาสาเหตุในการดับทุกข์ (สมุทัย) แล้วจึงตั้งจุดมุ่งหมายในการดับทุกข์ (นิโรธ) และดำเนินตามเส้นทางสู่ความดับทุกข์ (มรรค) คือสละ ละ ปล่อยวาง ไม่ยึดติดในใจด้วยอำนาจกิเลส

ทุกข์ถือเป็นความจริงอันประเสริฐข้อที่ 1 ในอริยสัจ 4 จึงเรียกว่าทุกขสัจ มี 11 อย่าง ได้แก่
1. ชาติ หมายถึง ความเกิด ความบังเกิด ความหยั่งลงเกิด เกิดจำเพาะ ความปรากฏแห่งขันธ์ ความได้อายตนะครบ ในหมู่สัตว์นั้นๆ ของเหล่าสัตว์นั้นๆ
2. ชรา หมายถึง ความแก่ ภาวะของความแก่ ฟันหลุด ผมหงอก หนังย่นเป็นเกลียว ความเสื่อมแห่งอายุ ความแก่หง่อมแห่งอินทรีย์ ในหมู่สัตว์นั้นๆ ของเหล่าสัตว์นั้นๆ
3. มรณะ หมายถึง ความเคลื่อน ภาวะของความเคลื่อน ความแตกทำลาย ความหายไป มฤตยู ความตาย ความทำกาละ ความทำลายแห่งขันธ์ ความทอดทิ้งซากศพไว้ ความขาดแห่งชีวิตินทรีย์ จากหมู่สัตว์นั้นๆ ของเหล่าสัตว์นั้นๆ
4. โสกะ หมายถึง ความแห้งใจ กิริยาที่แห้งใจ ภาวะแห่งบุคคลผู้แห้งใจ ความผาก ณ ภายใน ความแห้งผาก ณ ภายใน ของบุคคลผู้ประกอบด้วย ความพิบัติอย่างใดอย่างหนึ่ง ผู้ถูกธรรมคือ ทุกข์อย่างใดอย่างหนึ่ง กระทบแล้ว
5. ปริเทวะ หมายถึง ความคร่ำครวญ ความร่ำไรรำพัน กิริยาที่คร่ำครวญ กิริยาที่ร่ำไรรำพัน ภาวะของบุคคลผู้คร่ำครวญ ภาวะของบุคคลผู้ร่ำไรรำพัน ของบุคคลผู้ประกอบด้วย ความพิบัติอย่างใดอย่างหนึ่ง ผู้ถูกธรรมคือทุกข์ อย่างใดอย่างหนึ่งกระทบแล้ว
6. ทุกข์ (กาย) หมายถึง ความลำบากทางกาย ความไม่สำราญทางกาย ความเสวยอารมณ์ อันไม่ดีที่เป็นทุกข์เกิดแต่ กายสัมผัส
7. โทมนัส หมายถึง ความทุกข์ทางจิต ความไม่สำราญทางจิต ความเสวยอารมณ์อันไม่ดีที่เป็นทุกข์เกิดแต่ มโนสัมผัส (สัมผัสทางใจ นึกคิดขึ้นมา)
