อำเภอเกาะสมุยแบ่งเขตการปกครองออกเป็น 7 ตำบล 39 หมู่บ้าน ได้แก่
1. อ่างทอง (Ang Thong) 6 หมู่บ้าน
2. ลิปะน้อย (Lipa Noi) 5 หมู่บ้าน
3. ตลิ่งงาม (Taling Ngam) 5 หมู่บ้าน
4. หน้าเมือง (Na Mueang) 5 หมู่บ้าน
