กรมป่าไม้เริ่มทำการสำรวจเบื้องต้นป่าภูวัว เมื่อ พ.ศ. 2507 และพบว่าป่าแห่งนี้เป็นแหล่งต้นน้ำ ลำธารที่สำคัญ สภาพป่ามีความอุดมสมบูรณ์ และมีสัตว์ป่าชุกชุม สมควรที่จะจัดตั้งเป็นเขตรักษาพันธุ์สัตว์ป่าภูวัว ต่อมาในปี พ.ศ. 2508 จึงได้จัดส่งเจ้าหน้าที่อีกคณะหนึ่งเข้าไปทำการสำรวจอย่างละเอียด และทำการรังวัดหมายขอบเขตเพื่อทำการประกาศเป็นเขตรักษาพันธุ์สัตว์ป่าต่อไปแต่เนื่องจากสถานการณ์ไม่เป็นที่น่าไว้วางใจและพื้นที่ส่วนใหญ่เป็นพื้นที่แทรกซึมของผู้ก่อการร้าย การดำเนินงานต่าง ๆ จึงต้องหยุดชะงักลง จนถึงปี พ.ศ. 2517 เมื่อสถานการณ์ต่าง ๆ ภายในพื้นที่ป่าภูวัวคลี่คลายลง กรมป่าไม้จึงได้ดำเนินการต่อไปจนสามารถนำเรื่องเสนอขอความเห็นจากคณะรัฐมนตรีและตราพระราชกฤษฎีกากำหนดพื้นที่ป่าภูวัว ให้เป็นเขตรักษาพันธุ์สัตว์ป่าภูวัว โดยประกาศในราชกิจจานุเบกษา เล่ม 92 ตอนที่ 87 ลงวันที่ 2 พฤษภาคม 2518 มีเนื้อที่ประมาณ 186.5 ตารางกิโลเมตร หรือ 116,562 ไร่ อยู่ในท้องที่ตำบลหนองเดิ่น ตำบลบุ่งคล้า ตำบลโคกกว้าง อำเภอบุ่งคล้า ตำบลบ้านต้อง ตำบลโสกก่าม อำเภอเซกา ตำบลชัยพร อำเภอบึงกาฬ และตำบลท่าดอกคำ อำเภอบึงโขงหลง จังหวัดหนองคาย และในปี พ.ศ. 2533 ได้มีการรังวัดฝังหลักเขตรอบพื้นที่บริเวณใกล้เคียงกับหน่วยพิทักษ์ป่าถ้ำฝุ่น ซึ่งอยู่ในเขตป่าสงวนแห่งชาติดงภูวัว ยังมีความอุดมสมบูรณ์และมีสัตว์ป่าอาศัยอยู่ จึงได้ผนวกพื้นที่บริเวณนี้เข้าเป็นเขตรักษาพันธุ์สัตว์ป่าภูวัว เนื้อที่ประมาณ 8,100 ไร่ รวมมีเนื้อที่ทั้งสิ้น 124,662 ไร่
