พ.ศ. 2529 มหาวิทยาลัยธรรมศาสตร์ ได้ดำเนินตามแผนโดยการสร้างโรงพยาบาลขึ้น โดยศาสตราจารย์ คุณหญิงนงเยาว์ ชัยเสรี ดำรงตำแหน่งอธิการบดี มีคำสั่ง ที่ 747/2529 ลงวันที่ 25 สิงหาคม พ.ศ. 2529 แต่งตั้งคณะกรรมการจัดหาทุนสร้างโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ มีศาสตราจารย์สุธี นาทวรทัน เป็นประธานกรรมการ และ นายประมวล สภาวสุ รัฐมนตรีว่าการกระทรวงการคลัง ในขณะนั้น เป็นประธานกิตติมศักดิ์ ระดมทุนจากศิษย์เก่าทั้งในประเทศและต่างประเทศ โดย ณ ขณะนั้นมีผู้สนับสนันหลัก คือ ทายาท หม่อมราชวงศ์สุวพรรณ สนิทวงศ์ ธนาคารทหารไทย ศิษย์เก่ามหาวิทยาลัยธรรมศาสตร์ ประชาคมธรรมศาสตร์ ตลอดจนประชาชนทั่วไป
โดยพระบาทสมเด็จพระเจ้าอยู่หัวโปรดเกล้าฯ พระราชทานนามว่า "โรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ" และเสด็จพระราชดำเนินทรงประกอบพิธีวางศิลาฤกษ์ เมื่อวันที่ 3 พฤศจิกายน พ.ศ. 2529
พ.ศ. 2530 โรงพยาบาลเปิดให้บริการประชาชนเป็นครั้งแรก เมื่อวันที่ 5 ธันวาคม พ.ศ. 2530 เพื่อถวายเป็นราชกุศลเนื่องในมหามงคลสมัยที่พระบาทสมเด็จพระเจ้าอยู่หัวมีพระชนมพรรษาครบ 5 รอบ
พ.ศ. 2531 พระบาทสมเด็จพระเจ้าอยู่หัวทรงพระกรุณาโปรดเกล้าฯ ให้สมเด็จพระเทพรัตนราชสุดาฯ สยามบรมราชกุมารี เสด็จแทนพระองค์ทรงประกอบพิธีเปิดโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติอย่างเป็นทางการ ในวันที่ 29 มีนาคม พ.ศ. 2531 โดยในระยะแรกมีเพียง 2 อาคารที่เปิดให้บริการรักษาพยาบาลแก่ประชาชนทั่วไป คือ (1) อาคารหม่อมราชวงศ์สุวพรรณ สนิทวงศ์ เปิดให้บริการรักษาพยาบาลผู้ป่วยฉุกเฉิน ผู้ป่วยนอก และสำนักงานต่าง ๆ และ (2) อาคารธนาคารทหารไทย เปิดให้บริการรักษาพยาบาล
พ.ศ. 2533 ได้มีพระราชกฤษฎีกาจัดตั้งคณะแพทยศาสตร์[1] ในวันที่ 19 มีนาคม พ.ศ. 2533 โรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติจึงได้รับการรวมเข้ามาเป็นส่วนราชการหนึ่ง ซึ่งมีฐานะเทียบเท่าภาควิชา ของคณะแพทยศาสตร์ เช่นเดียวกับสถานวิทยาศาสตร์พรีคลินิก และสถานวิทยาศาสตร์คลินิก
พ.ศ. 2543 สภามหาวิทยาลัยธรรมศาสตร์ มีคำสั่งให้การบริหารโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติ เป็นอิสระโดยสังกัดสำนักงานอธิการบดีมหาวิทยาลัยธรรมศาสตร์[2] แยกออกจากคณะแพทยศาสตร์
ปัจจุบันนอกจากให้บริการวิชาการทางด้านการแพทย์และสาธารณสุขแก่ประชาชนโดยทั่วไปโรงพยาบาลธรรมศาสตร์เฉลิมพระเกียรติยังเป็นสถานที่ศึกษาดูงาน/ฝึกอบรม ดังนี้[3] เป็นแหล่งฝึกปฏิบัติงานสำหรับ คณะแพทยศาสตร์ มหาวิทยาลัยธรรมศาสตร์ คณะพยาบาลศาสตร์ มหาวิทยาลัยธรรมศาสตร์ และคณะสหเวชศาสตร์ มหาวิทยาลัยธรรมศาสตร์ และยังเป็นแหล่งศึกษาดูงานของโรงพยาบาลต่าง ๆ ทั้งในกรุงเทพฯ และต่างจังหวัด เช่น โรงพยาบาลเลิดสิน , โรงพยาบาลมหาราชนครเชียงใหม่ , คณะสาธารณสุข มหาวิทยาลัยธรรมศาสตร์ , โรงพยาบาลเซนต์หลุยส์ , โรงเรียนสวนกุหลาบวิทยาลัย นนทบุรี เป็นต้น
