ตามตำนานกล่าวว่าองค์พระธาตุเดิมสร้างมานานไม่น้อยกว่า 2,300 ปี โดยพระมหากัสสปะพร้อมพระอรหันต์ 500 องค์ ได้นำพระอุรังคธาตุของสมเด็จพระสัมมาสัมพุทธเจ้ามาบรรจุไว้ พระธาตุพนมเคยล้มทลายลงทั้งองค์ใน พ.ศ. 2518 เนื่องจากความเก่าแก่และถูกพายุฝนกระหน่ำต่อเนื่องกันหลายวัน ชาวไทยทั้งประเทศได้ร่วมกันบริจาคทุนทรัพย์บูรณปฏิสังขรณ์องค์พระธาตุขึ้นใหม่ตามแบบเดิม แล้วเสร็จใน พ.ศ. 2522 ในวันนี้พระธาตุพนมจึงยังคงตระหง่านงามอยู่เป็นที่เคารพสักการะของพุทธศาสนิกชนทั่วไป โดยเฉพาะอย่างยิ่งประชาชนในภาคตะวันออกเฉียงเหนือรวมไปถึงทางฝั่งลาวด้วย ในวันเพ็ญเดือน 3 ของทุกปี จะมีพุทธศาสนิกชนหลั่งไหลมาร่วมงานสมโภชพระธาตุพรมจากทั่วทุกสารทิศ
