I hereby declare that I have arranged for my own insurance policy which will provide coverage for any travelling mishaps, personal accidents, medical expenses (accidents and sickness) as well as emergency medical evacuation and repatriation throughout the entire period of my exchange in Malaysia. I understand that it is my responsibility to keep a valid insurance whilst studying in Malaysia.Should my stay in Malaysia be more than six (6) months, I understand that I will need to purchase insurance, upon my arrival, from a Malaysian based insurance company for the purpose of my visa application. The insurance company which I will purchase the insurance from shall be determined by the International Office of Universiti Sains Malaysia.
