การวิจัยเชิงปฏิบัติการนี้มีวัตถุประสงค์เพื่อพัฒนารูปแบบการดูแลและการฟื้นฟูสมรรถภาพของผู้ป่วยจิตเวชในชุมชนให้มีความเหมาะสมกับบริบทของพื้นที่และการมีส่วนร่วมของผู้เกี่ยวข้องเพื่อให้สอดคล้องกับสภาพปัญหาและความต้องการของผู้ป่วยด้วยการประยุกต์ใช้กระบวนการเรียนรู้แบบมีส่วนร่วม AIC) เป็นเครื่องมือในการวิเคราะห์สภาพปัญหา

ดำเนินการในพื้นที่รับผิดชอบของโรงพยาบาลสรรพยา กลุ่มตัวอย่างคือ ผู้ป่วยตเวชที่มีประวัติรับประทานยาไม่สม่ำเสมอ ขาดยา และมีอาการป่วยเรื้อรังมานานกว่า 2 ปี ขึ้นไป จำนวน 40 คน โดยให้ผู้มีส่วนเกี่ยวข้องเข้าร่วมกระบวนการเป็นผู้ร่วมตัดสินใจร่วมเรียนรู้ วางแผนและดำเนินการและติดตามประเมินผล เก็บรวบรวมข้อมูลและวิเคราะห์ข้อมูลเชิงคุณภาพ ดำเนินการระหว่างเดือนตุลาคม 2556 –มิถุนายน 2557
ผลการศึกษา พบว่า สามารถพัฒนารูปแบบการดูแลผู้ป่วยจิตเวชในชุมชนที่มีความเหมาะสม และแนวทางการดูแลผู้ป่วยจิตเวชทั้งในโรงพยาบาลและชุมชนที่ชัดเจน เป็นรูปธรรม โดยมีพยาบาลประจำโรงพยาบาลส่งเสริมสุขภาพตำบลทำหน้าที่เป็นผู้จัดการผู้ป่วย (Case Manager) เกิดกระบวนการสร้างความร่วมมือกับแกนนำชุมชน องค์กรปกครองส่วนท้องถิ่นให้การสนับสนุนงบประมาณ ส่งผลทำให้เกิดเครือข่ายดูแลผู้ป่วยจิตเวชในชุมชน อำเภอสรรพยา ที่มีความเข้มแข็ง สามารถบริหารจัดการการดูแลผู้ป่วยจิตเวชในชุมชนได้อย่างมีประสิทธิผล
ข้อเสนอแนะจากผลการวิจัยครั้งนี้ หน่วยงานที่เกี่ยวข้องในการดูแลผู้ป่วยจิตเวชในชุมชนสามารถนำรูปแบบการพัฒนาฯ ไปประยุกต์ใช้ให้เหมาะสมกับสภาพปัญหาของผู้ป่วยจิตเวชและบริบทชุมชน จะส่งผลทำให้ผู้ป่วยจิตเวชได้รับการดูแลอย่างมีคุณภาพ
