เช่น

- กำหนดขอบเขตของ Project ไม่ชัดเจน
- Programmer ไม่เข้าใจ Product feature ทำให้พัฒนาโปรแกรมไม่ถูกต้องไม่ตรงกับความต้องการของภาคธุรกิจ
และพบเป็นปัญหาในการทดสอบ
- ทางผู้ใช้งาน เข้าใจไม่ตรงกัน กับ ทีมพัฒนาโปรแกรม
- เนื่องจากไม่มีเอกสาร(FS ) แนะนำก่อนทำการ UAT ทำให้ ผู้ใช้งาน ทำการทดสอบไม่ถูกต้อง
