MP Sheet หรือ Maintenance Prevention Sheet คือแบบฟอร์มที่จัดทำขึ้นเพื่อรวบรวมข้อเสนอแนะ/ปัญหาในการดำเนินงานหรือการใช้งานเครื่องจักร/อุปกรณ์ภายในโรงงาน รวมถึงเครื่องจักร/อุปกรณ์ที่ติดตั้งแล้วเสร็จและใช้งานได้เป็นอย่างดี เพื่อใช้เป็นข้อมูลในการออกแบบและคัดเลือกเครื่องจักร/อุปกรณ์สำหรับโครงการต่างๆ ที่จะเกิดขึ้นในอนาคต ทั้งนี้ เพื่อเป็นการป้องกันและลดปัญหาต่างๆ ที่อาจจะเกิดขึ้นซ้ำได้อีก ซึ่งเป็นส่วนหนึ่งของการดำเนินงาน ตามระบบ TPM : EM Pillar
