การศึกษาปัจจัยที่มีความสัมพันธ์ต่อผลการตรวจชิ้นเนื้อที่มีการอักเสบเฉียบพลันหรือการแตกของผู้ป่วยที่ได้รับการผ่าตัดเอาไส้ติ่งออก ในโรงพยาบาลพิมาย จังหวัดนครราชสีมา นี้เป็นการศึกษาแบบ Retrospective research โดยศึกษาข้อมูลย้อนหลังจากเวชระเบียนในกลุ่มผู้ป่วยที่ได้รับการผ่าตัดเอาไส้ติ่งออก ในโรงพยาบาลพิมาย จังหวัดนครราชสีมา โดยมีวัตถุประสงค์ เพื่อศึกษาถึงปัจจัยที่มีความสัมพันธ์ต่อผลการตรวจชิ้นเนื้อที่มีการอักเสบเฉียบพลันหรือการแตกไส้ติ่ง คัดเลือกกลุ่มตัวอย่าง ตัวอย่างแบบเจาะจง (Purposive sampling) โดยมีคุณสมบัติ คือเป็นผู้ป่วยที่เข้ารับการรักษาที่โรงพยาบาลพิมาย ที่ได้รับการผ่าตัด เอาไส้ติ่งออก ในปีงบประมาณ 2554 – 2556 และมีผลการตรวจชิ้นเนื้อ ระบุว่าเป็น ไส้ติ่งอักเสบทั้งที่แตก และไม่แตก ได้กลุ่มตัวอย่างตรงตามคุณสมบัติจำนวน 147 คน เครื่องมือที่ใช้ในการทดลองในครั้งนี้ ได้แก่ แบบเก็บรวบรวมข้อมูล ผู้ป่วย และเวชระเบียนผู้ป่วย โรคไส้ติ่งอักเสบ โรงพยาบาลพิมาย จังหวัดนครราชสีมา ปี งบประมาณ 2554-2556 เครื่องมือที่ใช้ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้เชี่ยวชาญ จำนวน 3 ท่าน ใช้สถิติเชิงพรรณนา หาความถี่ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน ในการวิเคราะห์ข้อมูลข้อมูลทั่วไป ข้อมูลอาการ และอาการแสดง ข้อมูลการตรวจร่างกาทางห้องปฏิบัติการ โดยใช้สถิติ Spearman product moment correlation coefficient วิเคราะห์ความสัมพันธ์ ระหว่างข้อมูลอาการ และอาการแสดงข้อมูลการตรวจร่างกาย ผลการตรวจทางห้องปฏิบัติการ ผลการตรวจชิ้นเนื้อ ไส้ติ่งอักเสบ เฉียบพลัน และไส้ติ่งแตก
ผลการวิจัยพบว่า กลุ่มตัวอย่าง ส่วนใหญ่ เป็นเพศหญิง ร้อยละ 55.10 อายุส่วนใหญ่อยู่ในช่วง วัยผู้ใหญ่ตอนปลาย ช่วงอายุ 19-34 ปี ร้อยละ 46.94 สำหรับอาการนำมาคือ อาการปวดท้องน้อยข้างขวา (ร้อยละ 68.03)ส่วนใหญ่ไม่มีย้ายที่ปวด ร้อยละ 85.03 มีอาการร่วมที่นำมาคือ อาการไข้ (ร้อยละ 55.78)การตรวจร่างกายนั้นส่วนใหญ่แล้วพบ อาการกดเจ็บที่ท้องน้อยข้างขวา ( Tenderness at RLQ )ร้อยละ 93.88 การตรวจนับความสมบูรณ์ของเม็ดเลือด ของกลุ่มตัวอย่างส่วนใหญ่มีภาวะ Leucocytosis คือ Leucocyte Count > 100,000 / ลบ.มม ร้อยละ 93.88 ร่วมกับการมี PMN predominate พบ Neutrophil ≥ 75% ร้อยละ 80.27 ผลการตรวจชิ้นเนื้อไส้ติ่ง เป็น ไส้ติ่งอักเสบเฉียบพลัน (Acute appendicitis )ร้อยละ 80.95และ ไส้ติ่งแตก(Perforate appendicitis)ร้อยละ 19 .05 ระยะเวลาที่ปวด จนกระทั่งมาถึงโรงพยาบาล และระดับ Neutrophil มีความสัมพันธ์ กับ ผลการตรวจชิ้นเนื้อ ไส้ติ่งอักเสบเฉียบพลัน (Acute appendicitis ) และไส้ติ่งแตก(Perforate appendicitis) อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05
จากการศึกษาในครั้งนี้ นั้น พบว่าประวัติ ตรวจร่างกาย และตรวจพื้นฐานทางห้องปฏิบัติการ เป็นปัจจัยหลักสำคัญและเป็นสิ่งจำเป็น ในการวินิจฉัยโรคไส้ติ่งอักเสบ ระดับ Neutrophil เป็นปัจจัยที่สำคัญการวินิจฉัยและการวางแผนการรักษา เพราะจากผลการศึกษาพบว่า มีความสัมพันธ์ต่อผลการตรวจชิ้นเนื้อที่มีการอักเสบเฉียบพลันหรือการแตกของผู้ป่วยที่ได้รับการผ่าตัดเอาไส้ติ่งออกอย่างมีนัยสำคัญทางสถิติ
