ข้าพเจ้า นางสาวกนกกร นาถพัฒนกร เจ้าของบ้านเลขที่ 44/194 โครงการภัสสร 27 อ่อนนุช ขอให้บริษัท พฤกษา เรียลเอสเตท จำกัด (มหาชน) “บริษัท” จ่ายเงินค่าเสียหายเนื่องจากบ้านถูกโจรกรรมเมื่อวันที่ 26 มีนาคม 2557 ทำให้เกิดทรัพย์สินสูญหาย จากการเจรจาทั้งสองฝ่ายได้ตกลงยุติข้อร้องเรียนจึงได้มอบเงินช่วยเหลือแก่ข้าพเจ้า ข้าพเจ้าจึงตกลงทำบันทึกฉบับนี้ เพื่อเป็นหลักฐานว่า
1. ข้าพเจ้านางสาวกนกกน นาถพัฒน์กร ได้รับเงินจากบริษัทเป็นจำนวน .............................. บาท (..................................................) เป็นเช็คธนาคาร ...................................... เช็คเลขที่ ....................... ฉบับลงวันที่ ....................................... สั่งจ่าย “นางสาวกนกกน นาถพัฒน์กร” แล้วในวันที่ทำบันทึกฉบับนี้
2. ข้าพเจ้า ผู้รับเงิน/ผู้บันทึก ตกลงไม่ใช้สิทธิ์เรียกร้องเงิน หรือค่าเสียหายอื่นใดจากบริษัทอีกต่อไปและยุติการใช้สิทธิฟ้องคดี ทั้งทางแพ่งและอาญา ในกรณีดังกล่าว ต่อบริษัทและการร้องเรียนต่อสำนักงานคณะกรรมการคุ้มครองผู้บริโภค หรือหน่วยงานอื่นใดๆ หรือดำเนินการอื่นใดที่ก่อให้เกิดความเสียหายแก่บริษัท
