อาการของโรคกระเพาะทะลุ
อาการที่สำคัญของกระเพาะทะลุได้แก่
เกิดอาการปวดท้องอย่างรุนแรงและเฉียบพลัน เจ็บบริเวณลิ่มปี่ อาจจะร้าวไปไหล่
อายุที่พบบ่อย 48 ปี
ร้อยละ 29 ของผู้ป่วยจะมีประวัติเป็นแผลโรคกระเพาะ
ร้อยละ 20 ของผู้ป่วยมีประวัติการใช้ยาแก้ปวดกลุ่ม NSAID
ร้อยละ50ของผู้ป่วยจะมีอาการคลื่นไส้อาเจียน
การวินิจฉัย
แพทย์จะวินิจฉัยโรคนี้จากอาการที่สำคัญคือ อาการปวดท้องเฉียบพลัน ร่วมกับการตรวจร่างกายพบว่าชีพขจรจะเร็ว หากป่วยมานานจะมีความดันโลหิตต่ำ การตรวจร่างกายที่สำคัญคือท้องจะอืด เมื่อเคาะบริเวณตับจะพบว่าเคาะโปร่ง(ปกติจะเคาะทึบ) การวินิจฉัยที่สำคัญคือการ X-ray ปอดท่ายืนจะพบอากาศในช่องท้อง
สาเหตุของกระเพาะทะลุ
สาเหตุที่พบบ่อยได้แก่การติดเชื้อ H. pylori
การใช้ยาแก้ปวดกลุ่ม NSAID
การรักษากระเพาะทะลุ
การรักษาแบบไม่ผ่าตัด

โดยปกติการผ่าตัดจะได้ผลดีเมื่อผ่าตัดภายใน 8 ชั่วโมง หากเกิน 12 ชั่วโมงผลการผ่าตัดจะไม่ดี เนื่องจากกระเพาะทะลุจะมีโอกาสที่แผลทะลุจะหายได้เอง ประกอบกับผู้ป่วยมีความเสี่ยงสูงหากผ่าตัด ดังนี้ผู้ป่วยที่มีลักษณะดังต่อไปนี้จะใช้การรักษาโดยวิธีไม่ผ่าตัด
มีโรคประจำตัวเช่นโรคหัวใจ โรคปอด
ผู้ป่วยมีอาการมากกว่า 12 ชั่วโมงและอาการไม่เป็นมากขึ้นซึ่งเชื่อว่าแผลทะลุหายเองได้
วิธีการรักษา
จะใส่สายเข้าทางจมูกและดูดเอาน้ำย่อยออกให้หมด
ให้น้ำเกลือ
ยาแก้ปวด
ยาปฏิชีวนะ
จะต้องติดตามผู้ป่วยโดยใกล้ชิดหากอาการปวดท้องเป็นมากขึ้น หรือสัญญาณชีพไม่ดีจะทำการผ่าตัด
