โรคเบาหวาน โรคความดันโลหิตสูง หากขาดการควบคุมระดับน้ำตาลในเลือดและระดับความดันโลหิต อาจเกิดภาวะแทรกซ้อนต่ออวัยวะต่าง ๆ ที่สำคัญและเกิดความสูญเสียได้ การวิจัยเรื่องนี้มีวัตถุประสงค์เพื่อให้ผู้ป่วยสามารถควบคุมระดับน้ำตาลในเลือด ระดับความดันโลหิตได้ดีขึ้น ประชากรคือ ผู้ป่วยโรคเบาหวาน โรคความดันโลหิตในพื้นที่จำนวน 334 คน เลือกกลุ่มตัวอย่างแบบกลุ่มโดยใช้ปิงปองจราจร 7 สี จำนวน 208 คน เก็บรวบรวมข้อมูลระหว่างเดือน ตุลาคม 2556 - มีนาคม 2558 วิธีการดำเนินงานใช้รูปแบบการมีส่วนร่วมของชุมชน ได้แก่ อสม. ผู้นำชุมชน จิตอาสาและผู้ป่วย สร้างแรงจูงใจให้ผู้ป่วยในการเพิ่มพุนความรู้ ปรับพฤติกรรมสุขภาพโดยการใช้ปิงปองจราจร 7 สี และใช้กระบวนการกลุ่ม Selp help group วิเคราะห์ข้อมูลสถิติที่ใช้เชิงคุณภาพค่าเฉลี่ย ร้อยละ เปรียบเทียบความแตกต่างก่อนหลังด้วยสถิติ Paired t-test
