A 39 years old woman with low back pain, to rule out HNP.

Technique: MRI (1.5 Tesla) of lumbosacral spine was performed by sagittal T1W, T2W, STIR; axial T1W, T2W; coronal T2W and post Gadolinium T1W FS axial, coronal and sagittal. MR myelogram was done.

Findings: No prior study to comparison.
- The study reveals loss of normal lumbar lordotic curve.
- Evidence of irregularity surface of upper endplate and minimal anterior wedge-compression fracture of L4 vertebra is detected. No surrounded abnormal signal intensity in T1W, T2W and post Gadolinium enhancement is shown; chronic compression fracture is suspected. Mild narrowing of intervertebral disc space and low SI of L3/4 intervertebral disc are also observed; suspected dehydrate disc.
- Mild central bulging of L3/4 and L4/5 intervertebral discs without evidence of spinal cord and nerve root compression are noted.
- No abnormal SI of bone marrow and visible spinal cord are shown. The end of conus medullaris is located at L1 vertebra level. No paravertebral soft tissue mass is demonstrated.

Impression:
1) Minimal wedged compression fracture at anterior upper end plate of L4 vertebra; Old compression fracture is suspected.
2) Mild central HNP of L3/4 and L4/5 intervertebral discs without evidence of spinal cord and nerve root compression.
