ฟันทั้งปาก ,จานวนฟันผุ ,จานวนฟันต้องอุด ,จานวนฟันต้องถอนและบริเวณที่มีฟันผุเป็นส่วนใหญ่
การแบ่งระดับความรู้ เจตคติ และการปฏิบัติใช้วิธีการจัดกลุ่มแบบอิงเกณฑ์ ของ Bloom (1986:42)
แบ่งเป็น 3 ระดับดังนี้ คือ
คะแนน น้อยกว่า ร้อยละ 60 ระดับต่า
คะแนน ร้อยละ 60 -79 ระดับปานกลาง
คะแนน มากกว่าหรือเท่ากับ ร้อยละ 80 ระดับสูง
