CT. 017/2557 ลงวันที่ 11 พฤศจิกายน 2557
ทั้งนี้มีผลตั้งแต่วันที่ 17 พฤศจิกายน 2557 เป็นต้นไปและให้ถือว่าเอกสารฉบับนี้เป็นส่วนหนึ่งของสัญญา
