จากหลักสูตรการสอนของวิทยาลัยพยาบาลบรมราชชนนี ขอนแก่น ปีการศึกษา 2557 มีการจัดการสอนที่เน้นการเรียนรู้ด้วยตนเองจะเห็นว่านักศึกษาชั้นปีที่ 2 ซึ่งเป็นชั้นปีที่มีการเรียนการสอนที่มีเนื้อหาเริ่มเข้าสู่วิชาชีพพยาบาลเป็นปีแรก การปรับตัวในการเตรียมความพร้อมในการขึ้นฝึกปฏิบัติบนตึกผู้ป่วยจึงต้องมีความรู้ที่พร้อมจะดูแลผู้ป่วยเมื่อขึ้นฝึกปฏิบัติ ต้องมีการศึกษาหาความรู้มาก่อน มีการเรียนรู้ด้วยตนเองมากขึ้นเพื่อจะได้นำไปใช้ในการเรียนในชั้นปีที่สูงขึ้น เป็นการสร้างและเตรียมความพร้อมในการเรียนรู้ด้วยตนเอง จากการสังเกตพฤติกรรมของนักศึกษาชั้นปีที่ 2 วิทยาลัยพยาบาลบรมราชชนนี ขอนแก่น พบว่านักศึกษามีพฤติกรรมการเรียนรู้ด้วยตนเองน้อย ซึ่งนักศึกษาส่วนมากใช้เวลาในการทำธุระส่วนตัวมากกว่าการศึกษาค้นคว้าด้วยตนเอง เช่น การดูโทรทัศน์ เล่นอินเทอร์เน็ต ฟังเพลง อ่านหนังสือการ์ตูน คุยโทรศัพท์กับเพื่อน นอนหลับ เป็นต้น โดยจะเห็นได้ว่านักศึกษาชั้นปีที่ 2 ยังขาดความพร้อมในการเรียนรู้ด้วยตนเอง
ดังนั้นคณะวิจัยจึงได้สร้างเครื่องมือที่ช่วยในการส่งเสริมพฤติกรรมความพร้อมการเรียนรู้ด้วยตัวเองโดยประยุกต์จากทฤษฎีการตั้งเป้าหมายในรูปนวัตกรรมสมุดบันทึก “My Goals” การแก้ปัญหาเพื่อจะได้เป็นตัวกระตุ้นให้นักศึกษาชั้นปีที่ 2 มีการศึกษาหาความรู้ด้วยตนเองมากขึ้นเพื่อจะได้เตรียมความพร้อมเข้าสู่การเรียนในวิชาชีพทางการพยาบาล
