สมเด็จพระเทพรัตนราชสุดา ฯ สยามบรมราชกุมารี ได้ตามเสด็จพระบาทสมเด็จพระเจ้าอยู่หัว และสมเด็จพระนางเจ้าฯ พระบรมราชินีนาถ ไปทรงเยี่ยมราษฎรในถิ่นทุรกันดารทั่วทุกภาคของประเทศไทยตั้งแต่ยังเยาว์พระชันษา ทรงพบเห็นสภาพปัญหาและความเป็นอยู่ของประชาชน โดยเฉพาะเด็กและเยาวชนขาดแคลนอาหารที่จะบริโภค เป็นโรคขาดสารอาหาร และสุขภาพอ่อนแอ เจ็บป่วย ไม่แข็งแรง ขาดการศึกษา สภาพแวดล้อมเสื่อมโทรม ทรงมีพระราชหฤทัยมุ่งมั่นที่จะช่วยเหลือเด็กและเยาวชนในถิ่นทุรกันดารเหล่านี้ ซึ่งเป็นกลุ่มที่ได้รับผลกระทบมากที่สุดให้มีคุณภาพชีวิตที่ดีขึ้น

ดังนั้นเมื่อทรงสำเร็จการศึกษา จึงทรงเริ่มงานพัฒนาเด็กและเยาวชนในถิ่นทุรกันดารขึ้น ในปี พ.ศ. ๒๕๒๓ โดยทรงทดลองทำโครงการเกษตร เพื่ออาหารกลางวัน ในโรงเรียนตำรวจตระเวนชายแดน จำนวน ๓ โรง เพื่อพัฒนาภาวะโภชนาการและสุขภาพของเด็กที่อยู่ในท้องถิ่นทุรกันดาร ปัจจุบันการดำเนินงานการพัฒนาเด็กและเยาวชนในถิ่นทุรกันดาร ตามพระราชดำริ สมเด็จพระเทพรัตนราชสุดา ฯ สยามบรมราชกุมารี เป็นการพัฒนาแบบองค์รวม ที่ยึดเด็กและเยาวชนเป็นศูนย์กลางของการพัฒนาโดยผ่านกระบวนการเรียนรู้ โดยมีเป้าหมายในการดำเนินงาน ดังนี้

“ เพื่อให้เด็กและเยาวชนในถิ่นทุรกันดาร มีโภชนาการดี สุขภาพแข็งแรง มีความซื่อสัตย์ เสียสละ สามัคคี และเมตตาผู้อื่น มีความรู้และทักษะทั้งทางวิชาการและการอาชีพ ที่เป็นรากฐานของการพึ่งตนเองและพัฒนาท้องถิ่นได้ นำไปสู่การมีคุณภาพชีวิตที่ดีขึ้น และมีโอกาสได้รับการศึกษาในระดับสูงขึ้นตามศักยภาพของตนเองเป็นคนมีประสิทธิภาพดียิ่งขึ้น ”

ปัจจุบันมีพื้นที่เป้าหมายในการพัฒนาเด็กและเยาวชนทั้งในถิ่นทุรกันดาร โรงเรียนสอนศาสนาอิสลาม โรงเรียนพระปริยัติธรรม และโรงเรียนในพื้นที่กรุงเทพมหานคร ทั้งหมด ๗๗๖ แห่ง กระจายอยู่ทั้ง ๔ ภาคของประเทศ ประกอบด้วย
